In external piles, there is a lot of pain but not much bleeding. In the case of internal piles, there is discharge of dark blood. In some cases the veins burst and this results in what is known as bleeding piles.

Fissures are a little different. They are cracks around the anal sphincter. These usually heal on their own depending on what is causing them.

Fistula is described as an abnormal connection that forms between two organs or vessels that are lined with epithelial cells. In case of acute fissures, severe periodic pain may be felt after defecation. Anal fissures generally expand from anal opening and are usually positioned posterior in the midline.

Ayurveda suggests kshara soothra therapy for piles, fissure, and fistula. Kshara sutra is an ayurvedic medicated thread which is prepared by smearing a special linen thread with ayurvedic drugs. It takes about one month to prepare it. It is used for the treatment of anorectal diseases.
